In an Affidavit of Truth the "Affiant" (the person making the Affidavit of Truth) swears that what they are saying is true and correct, with their notarised signature as verification the Affiant is who they say they are.
Below is one example of one type of Affidavit of Truth for making a statement(s) of fact(s).
There are types that are addressed to specific parties, and may or may not have deadline dates to respond. (Those types should include other specific language not shown here).

Plus, Please be sure to include copies of evidence with your forms, and make a copy of everything for yourself before giving us a copy to maintain accurate records.
